That is so last century. Nowadays we don't promote good workers, because that means losing a good worker and getting a poor leader. Nowadays we spin the useless ones in the leadership merry-go-round. Poor leadership is still bound to happen, but the good workers save the day.

All depends where you are in a company hierarchy. Managers of employees should lead through example, expertise, developing their employees and empathy. Managers of managers do well through setting up a framework which will make their underling managers lead well and develop their employees.

Yeah, no. You become an example of your ideals, you listen to and observe your subordinates in order to improve your leadership and you use the force of authority when necessary. Sometimes some people won't be willing to accept your authority and these times the leader has to impose his/her authority, following the principle of the least necessary force. Or you can leave in your imaginary world where everyone will always act rationally and with good will.

As someone who was a manager, let me tell you how sometimes that philosophy backfires. Some people are absolutely fucking lazy. If you're the one leading by example they dont look up to you, they just let you do all the work. These are people who need to be babysat and pushed to work. And no, they don't get better over time, they need to be fired. But even though I say this, yeah I still do try to lead by example...

You are confusing a leader with a manager. A manager excels at micro and macro management whilst not actually participating in the daily business of his co-workers but managing it. A leader leads by example, showing his team that he holds up to his own high standards and motivating his co-workers by showing hard work and dedication. Don't confuse these two roles please, they are mutually exclusive
